Appointments
To schedule an appointment, please call our office. For your convenience, we have our new patient forms available for download. Print out and complete the forms, then bring to your first appointment.
Insurance
We accept most insurance plans; however, prior to an appointment, contact our office to verify acceptance of your plan. As a courtesy, we will bill your insurance. We do collect the patient portion for all treatment at time of service. Please speak with our Financial Coordinator for details.
